Duac Acne Gel

Duac Acne Gel contains Benzoyl Peroxide and Clindamycin in it. The combination used for the treatment of mild to medrate acne. Benzoyl Peroxide has antiseptic and keratolytic actions. On the other hand Clindamycin is an antibiotic and very effective against gram positive organism and many anaerobic bacteria.

Duac Formulation And Strength

Chlindamycin 1% benzoyl peroxide 5% gel:

Topical gel containing clindamycin (1.2% clindamycin phosphate ) at a concentration equivalent to 1 % w/w (10 mg/g ) and benzoly peroxide 5% w/w (50 mg/g ). A white to slightly yellow coloured gel.

Duac Dosage Administration

Duac Acne Cream is for topical use only.

Therefore, Duac Acne Cream should applied in a thin film over entire affected area once daily. Moreover, use Duac Cream For Acne in the evening after washing gently with a mild cleanser and fully drying. On the other hand, If Duac Gel dose not rub into the skin easily, too much is being applied. More importantly the hands should be washed after application. Patients may also use a moisturiser as needed.

If excessive dryness or peeling occurs, frequency of application should reduced or application temporarily interrupted for less than once daily dosing frequencies. Patients must advised that excessive application with not improve efficacy but it may increased the risk irritation.

Clindamycin 1 / benzoyl peroxide 5/ gel;

Two to five weeks of treatment may be required before a therapeutic effect is observed.

The safety and efficacy of Duac Gel For Acne have not studied beyond 12 weeks in acne vulgaris clinical trials. So, the prescriber should evaluate the benefit continuing treatment beyond 12 weeks of uninterrupted cream use.

Duac Dosage For Children

The safety and efficacy of Duac Gel have not established in children less than 12 years of age. Therefore Duac Cream not recommended for use in this population.

Duac Cream For Acne Contraindications

Duac Acne Cream contraindicated in patients who have demonstrated hypersensitivity to lincomycin , clindamycin, benzoyl peroxide or any components of the Dual Gel Ingredients. And patients with, or with a history of regional enteritis, ulcerative colitis , or antibiotic associated colitis (including pseudomembranous colitis).

Duac Gel Warning And Precautions

Contact with the mouth, eyes, lips, other mucous membranes or areas of irritated or broken skin should avoided. In case of accidental contact, rinse well with water. Moreover, during the first weeks of treatment, an increased in peeling and reddening will occur in most patients. Depending upon the severity of these side effects, patients can use a moisturiser. And temporarily reduce the frequency of application of Duac Gel Cream For Acne or temporarily discontinue use. However, efficacy has not established for less than once daily dosing frequencies. Concomitant topical acne therapy should used with caution because a possible cumulative irritancy may occur. Which sometimes may get severe, especially with the use of peeling, desquamating, or abrasive agents.

If severe local irritancy Like. severe erythema, severe dryness and itching, severe stinging burning ) occurs than Gel Duac should discontinued. As benzoyl peroxide may cause increased sensitivity to sunlamps should not used and deliberate or prolonged exposure to sunlight should avoided. When exposure to strong sunlight cannot avoided, patients should advised to use a sunscreen product and wear protective clothing. If a patients has sunburn, this should resolved before using Cream Duac. The product may bleach hair and coloured or dyed fabrics. Most importantly, avoid contact with hair, fabrics, furniture or carpeting.

Pseudomembranous Colitis

Pseudomembranous colitis has been reported with nearly all antibacterial agents, including clindamycin, and may range in severity from mild to life threatening, with an onset of up to severe weeks following cessation of therapy.

Although this is unlikely to occur with topically applied UAC, if prolonged or singificant diarrhoea occur or the patients experiences abdominal cramps, treatment should be discontinued immediately and the patient investigated further, as the symptoms may indicate antibiotic associated colitis.

Resistance To Clindamycin

Benzoyl peroxide reduced the potential for emergence of organisms resistant to clindamycin. However, patients with a recent history of systemic or topical clindamycin or erythromycin use are more likely to have pre existing anti microbial resistant Propionibacterium acnes and commensal flora.

Cross Resistance

Cross resistance has been demonstrated between clindamycin and lincomycin. Resistance to clindamycin is often associated with inducible resistance to erythromycin.

Duac Gel Drug Interactions

No formal drug drug interactions studies have conducted with DUAC gel. DUAC should not used in combination with erthromycin containing products due to possible antagonism to the clindamycin component. On the other hand, Clindamycin has shown to have neuromuscular blocking properties that may enhance the action of other neuromuscular blocking agents. Therefore, DUAC should used with caution in patients receiving such agents. Concomitant application of DUAC with tretinoin, isotretinoin and tazarotene should avoided since benzoyl peroxide may reduce their efficacy and increase irritation. On the other hand. If combination treatment required, the products should applied at different times of the day one in the morning and the other in the evening . Using topical benzoyl peroxide containing preparations at the same time as topical sulphonamide containing products may cause and facial hair to temporarily

Duac Use During Pregnancy And Lactation

No data on the effect of topical clindamycin or benzoyl peroxide on fertility in humans.

Pregnancy

No well controlled studies in pregnancy women treated with topical clindamycin benzoyl peroxide alone in pregnant women. Animal studies do not indicate direct or indirect harmful effects with respect to reproductive toxicity. No effect during pregnancy anticipate since systemic exposure to clindamycin. And similarly benzoyl peroxide low. However, DUAC should used during pregnancy only if the expected benefit justifies the potential risk to the foetus.

Lactation

Topical DUAC has not been studied during breast feeding.

Percutaneous absorption of clindamycin and benzoyl peroxide is low. However, not known whether clindamycin or benzoyl peroxide excreted in human milk after topical application. So, Clindamycin excreted in human milk following oral and parenteral administration.

DUAC should during lactation only if the expected benefit justifies the potential risk to the infant. Therefore, To avoid accidental ingestion by the infant if used during lactation, DUAC should not applied to the breast area.

Symptoms And Signs

Excessive application of DUAC may result in severe irritation. In this event, discontinue use and wait until the skin has recovered. Topically applied benzoyl peroxide not generally absorbed in sufficient amounts to produce systemic effects. Excessive application of topically applied clindamycin may result in absorption of sufficient amounts to produce systemic effects. Therefore, In this event of accidental ingestion of DUAC, gastrointestinal adverse reactions similar to those seen with systemically administered clindamycin may seen.

Treatment

Appropriate symptoms measures should taken to provide relief from irritation due to excessive topical application. Accidental ingestion should managed clinically or as recommended by the National Poisons Center, where available.

Duac Storage Instructions

Most importantly for the pharmacist should store it in a refrigerator at 2C to 8C temperature. On the other hand must not freeze the product. For patient using Duac must store within 25 C temperature. Protect the gel from direct sun light and heat. Always keep all the medicine out of the reach of the children.
