Prothaiden Special warning And Precautions

Suicide/Suicidal Thoughts Or Clinical Worsening
Depression is associated with an increased risk of suicidal thoughts, self-harm and suicide (suicide-related events). This risk persists until significant remission occurs. As improvement may not occur during the first few weeks or more of treatment, patients should be closely monitored until such improvement occurs. it is the general clinical experience that the risk of suicide may increase in the early stages of recovery. Patients with a history of suicide-related events, or those exhibiting a significant degree of suicidal ideation prior to commencement of treatment are known to be at greater risk of suicidal thoughts or suicide attempts and should receive careful monitoring during treatment. A meta-analysis of placebo-controlled clinical trials of the antidepressant drugs in adult patients with psychiatric disorders showed an increased risk of suicidal behavior with antidepressants compared to placebo in patients less than 25 years old. close supervision of patients and in particular, those at high risk should accompany drug therapy especially in early treatment and following dose changes. Patients (and caregivers of patients) should be alerted about the need to monitor for any clinical worsening, suicidal behavior or thoughts, and unusual changes in behavior and to seek medical advice immediately if these symptoms present. Avoid use in patients with a history of epilepsy, thyroid disease, mania, or urinary retention and in those with narrow-angle glaucoma or symptoms suggestive of prostatic hypertrophy. patients with hepatic impairment and those undergoing electroconvulsive therapy should also avoid dosulepin use. The elderly are particularly liable to experience adverse effects with tricyclic antidepressants (TCAs), especially agitation, confusion, delirium, and postural hypotension.
PRECAUTIONS
Dosulepin is associated with high mortality in overdose. There is a low margin of safety between the (maximum) therapeutic dose and potentially fatal doses. The onset of toxicity occurs within 4-6 hours.
  • A limited number of tablets should be prescribed to reduce the risk from overdose for all patients and especially for patients at risk of suicide.
  • A maximum prescription equivalent to two weeks supply of Prothiaden 75 mg/day should be considered in patients with increased risk factors for suicide at the initiation of treatment, during any dosage adjustment, and until improvement occurs. Avoid concomitant medications which may increase the risk of toxicity associated with Dosulepin
  • Patients should advise to store the tablets securely, out of sight, and reach of children.
  • In cases of overdose, patients should seek immediate attention.
Tricyclic antidepressants potentiate the central nervous depressant activity of alcohol.

Prothiaden Tablets Interactions

Dosulepin should not give concurrently with a monoamine oxidase (MAO) inhibitor or within 14 days of ceasing such treatment. The concomitant administration of Dusulepin and selective serotonin reuptake inhibitors (SSRI) should avoid since an increase in plasma tricyclic, antidepressant levels have reported following the co-administration of some SSRI. Dosulepin may alter the pharmacological effect of some concurrently administered drugs including CNS depressants such as alcoholic analgesics. On the other hand, the effect of these will potentiated as will the effects of adrenaline and noradrenaline. Anesthetics given during tri/tetracyclic antidepressant therapy may increase the risk of arrhythmias and hypotension. Meanwhile, Dosulepin has quinidine like actions on the heart. Therefore, for this reason, its concomitant use with other drugs that may affect cardiac conduction e.g. bethanidine, debrisoquine, guanethidine may be reduced by Dosulepin. It is advisable to review all antihypertensive therapy during treatment with tricyclic antidepressants. There is an increased risk of postural hypotension when tricyclic antidepressants are given with diuretics. On the other hand, Tricyclic antidepressants may also antagonize the anticonvulsant effect of antiepileptics (convulsive threshold decreased). Meanwhile, Barbiturates may decrease and methylphenidate may increase the serum concentration of Dosulepin and thus affect its antidepressant action. There is no evidence that Dosulepin interferes with standard laboratory tests.

Prothiaden Tablets Use In During Pregnancy And Lactation

Pregnancy
The safe use of Dosulepin during pregnancy and lactation has not been adequately studied. However, withdrawal symptoms including insomnia, irritability and excessive perspiration have reported in neonates whose mother received tricyclic antidepressant treatment during the third trimester of pregnancy. Meanwhile, use in pregnant women, nursing mothers, and women who may become pregnant requires that possible benefits weighed against possible effects on mother and child.
Lactation
Dosulepin excreted in human milk. So most importantly breastfeeding mothers should discontinue during treatment with Dosulepin.
Prothiaden Effects On Driving And Use Machines

Prothiaden Overdosage Symptoms And Treatment

  • Tab Prothiaden Overdose Symptoms
Deaths may occur from overdosage with this class of medicines. Multiple drug ingestion including alcohol is common in deliberate tricyclic antidepressant overdose. The onset of toxicity occurs within 4 to 6 hours after tricyclic antidepressant overdosage. Meanwhile, symptoms of overdose include dry mouth, excitement, ataxia, drowsiness, unconsciousness, muscle twitching, convulsions, widely dilated pupils, hyperreflexia, sinus tachycardia, cardiac arrhythmias, hypotension, hypothermia, depression of respiration, visual hallucination, delirium, urinary retention, paralytic ileus, and respiratory or metabolic alkalosis. Patients investing >5 mg/kg should seek immediate medical attention. Most importantly, all children ingesting Dosulepin should assess by a physician.
  • Prothiaden Tab Overdose Treatment
A clear airway and adequate ventilation should ensure. On the other hand, hypoxia and acid-base imbalances should correct by assisted ventilation and intravenous sodium bicarbonate as appropriate. Therefore do not give flumazenil to reverse benzodiazepine toxicity in mixed overdoses. The use of activated charcoal should consider as the preferred initial means of reducing absorption in patients presenting within 2 hours of ingestion. However, repeated gastric intestinal aspiration or repeated administration of activated charcoal may remove drugs and metabolites excreted into the gut via the bile. On the other hand, the benefit of gastric lavage is uncertain and the technique should avoid in any patient with an impaired airway. Meanwhile when the patient unconscious or the cough reflex depressed the lungs should protect by a cuffed endotracheal tube. Therefore, Forced diuresis not recommended. Blood pressure, pulse, and cardiac rhythm should monitor for at least 6 hours after ingestion. Arrhythmias best treated by correcting hypoxia and acid-base disturbances. Specialists’ poisons advice should sought before using any antiarrhythmic agents as these may exacerbate arrhythmia. In cases of cardiac arrest, persist with prolonged CPR. Meanwhile, Convulsions should control with intravenous diazepam or lorazepam. Most importantly, bed rest advised even after recovery.
Tab Capsules Storage Instructions
Most importantly, always keep all the medical products, tablets, injection, syrups, and creams out of the reach of the children. On the other hand, protect the tab and caps from direct sunlight and heat. Store it in a cool and dry place meanwhile protect it from moisture.
