Contraindications Details

Meloxicam (Xobix) is contraindicated in patients with known hypersensitivity to the medicine. It should not given to patients who have experienced asthma, urticaria or allergic type reactions after taking aspirin or other NSAIDs. Severe, rarely fatal, anaphylactic like reactions to NSAIDs have reported in such patients.

Advanced Renal Disease

In late pregnancy as with other NSAIDs Meloxicam should be avoided because it may cause premature closure of the ductus arteriosus.

Warnings And Precautions

General

Meloxicam cannot be expected to substitute for corticosteroids or to treat corticosteroid insufficiency. Abrupt discontinuation of corticosteriods may lead to disease exacerbation. Meanwhile patients on prolonged corticosteroid therapy should have their therapy tapered slowly if a decision is made to discontinue corticosteroids. The pharmacological activity of the capsules in reducing inflammation and possibly fever may diminish the utility of these diagnostic signs in detecting complications of presumed non-infections, painful conditions.

Pregnancy
Teratogenic Effects

Meloxicam caused an increased incidence of septal of the heart, a rare event, at an oral dose of 60mg/kg/day (65.5-fold the human dose at 15 mg/kg/day for a 50 kg adult based on body surface area conversion) and embryo lethality at oral doses. 5 mg/kg/day (5.4-fold the human dose, as noted above) when rabbits were treated throughout organogenesis. It was not teratogenic in rats up to an oral dose of 4 mg/kg/day ( approximately 2.2-fold the human dose, as noted above) throughout organogenesis, An increased incidence of stillbirths was observed when rats were given oral doses. 1 mg/kg/day throughout organogenesis. The medication crosses the placental barrier pregnancy only if the potential benefits justifies the potential risk to the fetus.

Nursing Mothers;

Studies of Meloxicam excretion in human milk have not been conducted: however, it was excreted in the milk of lactating rats at concentrations higher than those in plasma. Pendiatric use;

Safety and effectiveness in pediatric patients under 18 years of age have not been established.

Xobix Adverse Reactions

Gastrointestinal (Gl) adverse events the most frequently reported adverse events in all treatments groups across medication trials.

The adverse events reported include abdomen pain, diarrhoea, flatulence, nausea, oedema, dizziness, headache, skin rash, insomnia, flu like syndrome, etc. as with NSAIDs, higher doses of the medicine ( e.g chronic daily 30 mg dose) associated with an increased risk serious GI events. Xobix Side Effects

The following adverse events which may be causally related with the administration of the medicine have been reported. The frequencies given below are based on corresponding occurrences in clinical trials, regardless of any causal relationship. The information based on clinical trials involving 3750 patients who have treated with daily oral dose of 7.5 mg or 15 mg tablet over a period of up to 18 months ( mean duration of treatment: 127 days). Gastrointestinal:

Dyspepsia, nausea, vomiting, abdominal pain, constipation, flatulence, diarrhoea (more frequent than 1%). Transitory abnormalities of liver function parameters e.g. raised transaminases or bilirubin eructation. Oesophagitis, gastro duodenal ulcer, occult or macroscopic gastrointestinal bleeding (between 0.1% and 1%). Gastro intestinal perforation, colitis, hepatitis, gastritis (less frequent than 0.1%).

Hematological:

Anemia (more frequent than 1%). Disturbances of blood count, including differential white cell count, leukopenia and thrombocytopenia. Concomitant administration of a potentially myelotoxic drug, in particular methotrexate, appears to be a predisposing factor to the onset of a cytopenia (between 0.1% and 1%).

Dermatological:

Pruritus, skin rash (more frequent than 1%): stomatitis, urticaria (between 0.1% and 1%): photosensitization, on rare Occasion’s bullous reactions, erythema multiforme. Stevens Johnson syndrome. Toxic Epidermal Necrolysis may develop (less frequent tha0.1%).

Respiratory:

Onset of acute asthma has reported (less frequent than 0.1%) in certain individuals following the administration of aspirin or other NSAIDs, including Meloxicam.

Central Nervous System:

Lights-headedness, headache (more frequent than 1%): vertigo, tinnitus, drowsiness (between 0.1% and 1%): confusion and disorientation (less frequent than 0.1%).

Cardiovascular:

Oedema (more frequent than 1%): increase of blood pressure, palpitations, flushes (between 0.1% and 1%).

Vision Disorders:

Conjunctivitis, visual disturbances including blurred vision (less frequent than 0.1%).

Hypersensitivity Reactions:

Angio-oedema and immediate hypersensitivity reaction including anaphylactoid / anaphylactic reactions (less frequent than 0.1%).

Xobix 7.5 MG 15 MG Tab Dosage Administration

The lowest dose of Meloxicam should sought for each patient. For the treatment of osteoarthritis the recommended starting and maintenance dose of is 7.5 mg once daily. Some patients may receive additional benefit by increasing the dose to 15 mg once daily. The maximum recommended daily dose of 15 mg. It may taken without regard timing of meals.

Rheumatoid arthritis:

15 mg / day. According to the therapeutic response, the dose may reduced to 7.5 mg /day.

Ankylosing Spondylitis:

Xobex 15 mg / day. In patients with increased risk of adverse reaction. Start treatment at the dose of 7.5 mg/day. However In dialysis patient with severe renal failure: the dose should not exceed 7.5 mg / day. On the other hand, maximum recommended daily dose of Meloxicam is 15 mg. Meanwhile, As a dosage for use in children has yet to established. However, usage should restricted to adult. Overdosage Symptoms & Treatment

Symptoms following acute NSAID overdose usually limited to lethargy, drowsiness, nausea, vomiting, and epigastric pain, which are generally reversible with supportive care.
